I, as parent/guardian of of the camper listed above (“Child”), hereby assume all risks and hazards incidental to the conduct of the activities of Friendship Smile and transportation to and from the activities. I am voluntarily allowing my Child to attend and participate in Friendship Smile programs and activities. I acknowledge the risks of COVID-19 as set out herein. I and my Child voluntarily assume the risk that my Child may be exposed to or infected by COVID-19 while attending and participating in Friendship Smile programs and activities such exposure or infection may result in personal injury, illness, permanent disability, and/or even death. I understand and acknowledge that given the unknown nature of COVID-19, it is not possible to fully list each and every individual risk of contracting COVD-19. I acknowledge that I am making an informed assumption of all COVID-19 related risks associated with my Child’s attendance of and participation in Friendship Smile program and activities. I have relied on my own judgement as to whether to undertake the risks. I hereby release and shall defend, indemnify, and hold harmless releases from every claim and any liability that I or my child may allege against releasees (including reasonable legal fees and costs) as a direct or indirect result of injury or death to me or my child because of my child’s participation in any Friendship Smile program or activities, whether caused by the negligence of releasees or others to the maximum extent permitted by law. I promise not to sue releasees on my behalf or on behalf of my child regarding any claim arising from or related to my child’s participation in any Camp for All or Friendship Smile program. I acknowledge that, by signing this document, I am releasing Friendship Smile and their representatives, agents, employees, volunteers, members, sponsors, promoters, and affiliates (collectively “releases”) from liability, and that I am giving up substantial legal rights. This release form is a contract with legal and binding consequences and it applies to all activities in which my child engages at Camp for All and with Friendship smile regardless of whether such activity is a part of a formal program. I have read this release carefully before signing. I understand what this release means and what I am agreeing to by signing. I understand that no insurance coverage for participants in these activities is provided by Friendship Smile
